Abstract Details

Purpose:

To analyze reasons, manifestations and treatment outcomes in patients with expulsive hemorrhage (EH) during phacoemulsification.

Setting:

IRTC Eye Microsurgery Ekaterinburg Center, Ekaterinburg, Russia

Methods:

Retrospective analysis of EH during small incision (1,9- 2,75mm) cataract surgery was performed. There were 14 eyes operated from 2008 till 2014 – 0.01% of cataract operations. Mean age - 75±3.6 years. Pre-op visual acuity was from light perception to 0.75; AL from 21.08 to 32.08 mm. Glaucoma was present in 4 eyes; in 3 eyes IOP was over 21 mm Hg, in 1 case combined surgery was performed. In 1 eye there was lens subluxation, in 4 eyes – grade 5 cataract density. EH grade 1 was in 6 eyes, grade 2 – 1, grade 3 – 7.

Results:

In remote post-op period up to 1 year visual acuity was 0.01 in 1eye with haemophtalmia, from 0.2 to 0.55 in 7 eyes, more than 0,6 in 6 eyes. In 93% cases (13 eyes) we observed good functional results, 1 patient with haemophtalmia refused from vitrectomy. IOP was normal in all eyes. As during phaco in 7 eyes anterior vitrectomy and in 9 eyes sclerotomy were performed, in 2 patients due to intraoperative transition to the laryngeal mask, in post-op period there were no need in additional revision.

Conclusions:

Use of small self-sealing incisions in cataract surgery results in reduction of negative functional results in case of EH. EH grades 1-2 occur more frequently during phaco. There is no need in additional revision of anterior segment if an adequate teсhnique is used at final steps of operation in case of EH.
