The analysis of clinical-economic effectiveness of toric IOL AcrySof Toric implantation during phacoemulsification cataract surgery in patients with initial corneal astigmatism

Abstract Details

Purpose:

The comparative assessment of clinical-economic effectiveness of toric and aspheric IOLs after the phacoemulsification among the patients with initial corneal astigmatism.

Setting:

Primorsky center of Eye Microsurgery. Russia,Vladivostok

Methods:

96 patients (105 eyes) with corneal astigmatism between 1,0 and 4,5 D were implanted with t IOLs during phacoemulsification. IOLs Acrysof Toric SA60TT (USA) were implanted in 51 patients (55 eyes) (the first group). IOLs Acrysof IQ SA60AT were implanted in 45 patients (50 eyes) (the second group). The clinical-economic analysis was made by using ' Cost-effectiveness analysis' (CEA) and ''Cost-utility analysis'' (CUA) methods. The difference between assessment of sight quality before cataract surgery and in one month later was as the unit of '' utility''. The assessment of vision quality was made by using the questionnarie VF-14 QOL.

Results:

The ''effectiveness'' index was considerably hire among patients who were implanted with toric IOLs (0,59+/- 0,11) comparing to spheric IOL (0,25+/-0,11). This difference was statistically positive (P=0,0001). There were no reliable differences accordinly the ' Cost-effectiveness'' index between two patients groups (P=0,81). In the first group the index was 21923,93+/-624,3 , in the second group it was 22572,85+/-436,6. The '' utility'' indexes increased more significantly (P=0,0001) in the first group (1678,88+/-1,99) comparing to the second group (18,55+/-0,97). The' Cost-effectiveness'' indexes are significantly lower in group of patients with toric IOLs - 211,85+/- 7,02 than with aspheric lenses - 268,11+/-15,29.

Conclusions:

The phacoemulsification cataract surgery with toric IOL Acrysof Toric implantation in patients with initial corneal astigmatism is not only one that is more effective for reaching high-level visual functions without correction but also one that is significantly more expensive. However, in case of toric IOL implantation, an additional economic spending are fully justified because of their life quality increasing.
