VITAL XL: the ultimate clinical keratoconus classification

Abstract Details

Purpose:

To report an improved and simplified individualized keratoconus descriptive classification coding system that by design incorporates 11 important clinical parameters that are necessary in choosing the right treatment for a particular keratoconic eye.

Setting:

AthensVision Eye Institute, Athens, Greece Hellenic Eye Bank Demokritos Gregoris Georgariou, Appleye Ltd.

Methods:

An individualized metric system was developed that allows separate grading of each targeted parameter in order to construct an algorithm that allows individualized surgical treatment options similar to the TNM staging system dictating individualized approach in cancer treatment.

Results:

Patients age, better or worse seeing eye, BSCVA with glasses up to 6dp astigmatism and subjective Va with adaptive optics or RGP lens, minimal pachymetric thickness, presence or not of scaring in the visual axis and topographic stability is recorded. We then record the theoretical corneal wavefront driven ablation volume to a total ablation zone of 8,50mm as a function of topographic abnormality and measured refractive error (The ORK-Schwind Excimer laser platform software was used) . Finally we record the AC depth (critical for utilizing Phakic IOL's) and total Axial Length (that correlates with postkeratoplasty refractive error). Example (24)' Vi21T2CSA3L23.

Conclusions:

the Vital-XL classification or rather coding system provides 11 important clinical keratoconus and anatomic features that we believe are enough to calculate an individualized surgical treatment algorithm. FINANCIAL DISCLOUSRE: NONE
