Abstract Details

Purpose:

To find a novel way to prevent endothelial tear/detachment during phacoemulsification in advanced pseudoexfoliation combined with diabetes mellitus undergoing phacoemulsification.

Setting:

Department of Ophthalmology, Rzgary Teaching Hospital, Kurdistan Regional Governorate, Erbil-Iraq, Sep, 2013 to Feb, 2015.

Methods:

After drop and subconjunctival anesthesia, the incisions done according to the groups. Group A, clear corneal incision for the site port and the main incision done. Group B, both incisions performed at the limbus. Group C, scleral tunnel incision for the both incisions. Then, the pupil dilated bimanually followed by capsulorhexis and hydrodissection, then primary vertical chop-stop done for almost all cases in one session (without re-insertion of the probes). Followed by coaxial IA and foldable lens insertion. In the tear or detachment cases; the anterior chamber formed with air for re-attaching the tear or the detachments

Results:

Comparison has been carried out after doing more than 5000 phacoemulsification. A total 83 eyes of 43 patients were taken in this study. Group A: First 15 cases; in 11 cases detachment happened for about 1- 1.5mm but only tear at the site ports. Group : 21 cases, endothelial tear happened about less than 1mm; in 12 but no detachment . Group C: 47 cases, neither tear nor detachment happened in almost all cases The data analyzed with SPSS 20 and the P-value was (0.006) for the Group C

Conclusions:

The study has shown that scleral tunnel for both the main incision and the side port provides a novel approach to prevent endothelial tear/detachment in combined pseudoexfoliation and diabetic mellitus during phacoemulsification.
