Interest of scleral lenses in challenging cornea

Purpose:

To determine the efficacy, safety and ease of scleral lens ( Misa Lens from Microlens, Neetherlands ) fitting in the management of very irregular corneas

Setting:

Cabinet d"ophtalmologie Dr MEKKI, 13 Bis Ben Kara Mohamed El Mansour, El Biar, Algiers

Methods:

Single center prospective and descriptive study of 100 consecutive patients with irregular cornea (including 51 stage IV keratoconus, 30 pellucid marginal degeneration,3 keratoglobus, 2 corneal scar , 1 keratoconus with failed corneal ring segment implant,2 post-lasik corneal ectasia , 1 irregular cornea after radial keratotomy and 10 irregular corneal graft) in whom rigid gas permeable contact lens failed or pending keratoplasty.

Results:

All patients were successfully fitted with Misa scleral lens whatever the degree of corneal ectasia. Mean visual acuity with scleral lens was 20/25, mean continuous time wear was 8 hours, 10 patients had conjunctival redness which disappeared after scleral lens exchange.

Conclusions:

Scleral lens is a very attractive solution in the management of challenging irregular corneas.
