Safety and efficacy of INTACS intracorneal ring segment implantation to improve best corrected visual acuity in children with keratoconus

To evaluate the safety and clinical outcomes of INTACS intracorneal ring segment (ICRS) implantation that was performed in pediatric keratoconic patients to enhance best corrected visual acuity (BCVA).

This is a case series study that included pediatric patients aged less than 14 years old with keratoconus and poor BCVA who underwent ICRS implantation. INTACS ICRS were inserted according to our topography-guided nomogram using Intralase Femtosecond. Records were reviewed and data were collected preoperatively, at the 6-month follow-up visit after ICRS implantation, and at later follow up visits.


4 patients (6 eyes) were included in this study. The patients age ranged from 9 to 14 years old. The follow up period varied from 6 months up to 6 years. The BCVA improved following ICRS insertion (Mean preoperative CDVA was 0.44 ±0.32 preoperatively vs 0.83 ±0.15 postoperatively). Also, uncorrected visual acuity (UCVA) increased significantly in all patients (Mean preoperative UCVA was 0.26 ±0.32 preoperatively vs 0.8 ±0.24 postoperatively on decimal scale). The mean spherical equivalence decreased from -4 ±2.33 D to -1.4 ±0.80 D. Keratometry readings remained stable after ICRS insertion: at last follow up visit, mean Kflat was 43 ±5.22 D vs. 43.78ḟ1.94 D preoperatively and mean Ksteep was 46.78ḟ5.52 D vs. 46.6 ḟ1.54 D preoperatively. All patients tolerated the surgery well with no intraoperative or postoperative complications.


INTACS ICRS implantation is a safe and effective procedure for visual rehabilitation in children with keratoconus with poor BCVA. When compared to our results in adults patients, INTACS ICRS implantation seems to result in better outcomes.
