Cataract surgery on a Marfan syndrome patient

Purpose:

To describe the surgical difficulties and challenges on an ectopic lens.

Setting:

Departmnt of Ophthalmology, Medical University of Pécs, Hungary

Methods:

A 10 year old girl with ectopic lens due to Marfan syndrome was operatied in general anaesthesia. Capsulorhexis was performed under cohesive OVD, followed by careful hydrodissection, and irigation-aspiration of the lens material. A Cionni capsulo tension ring was inserted and sutured to th eciliary sulcus, to center the capsurar bag. A posterior CCC combined with limited vitrectomy was performed before implantation of the PC-IOL into the capsular bag.

Results:

A well centered IOL and dramatically increased VA was seen already on the first postoperative day, and remained during the follow- up of one year.

Conclusions:

Safe and successful cataract surgery can be performed with the help of a Cionni ring on an ectopic lens. Even PCCC and vitrectomy can be performed if needed due o the patient age.
