To investigate the safety and effectiveness of the Artiflex iris-claw phakic lens (Ophtec, Groningen, Netherlands) for correction of moderate-to-high myopia and astigmatism in adults after a 12 month follow-up.

12 month follow-up prospective study of 94 consecutive eyes implanted with the Artiflex phakic IOL for the correction of myopia and astigmatism. Parameters studied: Uncorrected visual acuity (UCVA) and best spectacle corrected visual acuity (BSCVA) pre and postoperative, mean manifest refraction spherical equivalent (MRSE) pre and postoperative, endothelial cell counts pre and postoperative and temporal and nasal critical distances after implantation. All patients had stable refractive error for more than 12 months, anterior chamber depth > 3,0 mm, endothelial cell count > 2200 cels/mm2 and no other ocular pathology. The IOL model was chosen according to the level of refractive astigmatism: patients with refractive astigmatism <1.50 diopters (D) were implanted with the Artiflex spherical model, for astigmatism ? 1.50 D we choose to implant the Artiflex toric model. All surgeries were performed under topical/local anesthesia, through a 3.0 mm corneal incision and an iridectomy was performed intraoperatively.
A total of 94 eyes were analysed: 51 patients with Artiflex spherical model and 43 patients with the Artiflex toric model. The overall preoperative mean spherical equivalent (MRSE) was -10,53 D, with a mean sphere of -9,50 D and a mean refractive cylinder of -2,27D. No subjects lost ?2 lines BSCVA. Mean UCVA postoperative was 0,95 ± 0,26; mean BSCVA postoperative was 1,10 ± 0,24. Postoperative mean spherical equivalent was -0,125 D. The overall mean percentage change in central endothelial cell density 12 months after surgery was -4,71 ± 0,92 %. No pupillary block, corneal decompensation or ocular hypertension were observed. We observed iris pigment precipitates in 21 % of patients at the end of the follow-up period.
The Artiflex phakic IOL provided excellent refractive correction and predictability. The wide range of spherical and cylinder correction provided an excellent range of correction for high refractive defects.
