to present the technique of small capsulotomy as a preliminary stage in capsulorhexis in intumescent white cataract surgery.

18 patients with white, intumescent cataract underwent (1) trypan blue injection to visualize the anterior capsule (2) dispersive viscoelastic insertion (3) quick small central radial capsulotomy (4) washout of the liquefied cortex (5) continuous curvilinear capsulorrhexis (CCC) and (6) phacoemulsification. The intraoperative behavior of anterior capsule was recorded.
the intentional multiple small radial cuts of the anterior capsule remained small and allowed the CCC. In 2 patients with marked fibrosis of the anterior capsule, where the capsulorhexis was not possible, wide capsulotomy was performed and phacoemulsification was completed without sequelae.
The intentional small capsulotomy with multiple radial cuts as a preliminary stage in CCC in white intumescent cataracts relaxes the anterior capsule and prevents large unexpected radial tears of the initial capsulotomy as well as the sudden radialization of the CCC, thus providing a safer white cataract surgery.
