To assess and to compare the centration of the treatment zone after ReLEx and Femtolasik procedure.

A retrospective review of 69 myopic eyes of 36 patients who underwent ReLEx was compared to a matched group of myopic Femtolasik eyes. The procedures were performed by a single surgeon using VisuMax? platform consisting of VisuMax femtosecond laser and MEL 80 excimer laser (Carl Zeiss Meditec AG/Germany). The mean spherical equivalent of the correction in the ReLEx group was -5.7 D (range -10.0 to -1.25 D) and the mean absolute cylinder was 0.79 D cyl (range 0 to 4 D cyl) . The mean spherical equivalent of the correction in the Femtolasik group was -4.88 D (range -10.0 to -0.75 D) and the mean absolute cylinder was 0.91 D cyl (range 0 to 4.5 D cyl). Pentacam? (Oculus, Germany) was used for pre- and postoperative topography and pachymetry. In all ReLEx cases the intraoperative centration was aimed at the first Purkinje image while the patient was observing a blinking light. In the Femtolasik group the eye tracker was logged on at the pupil centre. The position and the centration of the treatment zone was estimated pachymetrically by the distance of the thickest point on pachymetry differential map (which corresponds to the maximum refractive power and the centre of the lenticule/ablation mass) from the topographical centre of the entrance pupil and the corneal apex.
In ReLEx cases the mean decentration of the lenticule from the centre of the entrance pupil was 0.326 mm (0.022 to 1.062 mm). In Femtolasik cases this value was 0.452 mm (0.020 to 1.040). In relation to the corneal apex the decentration of ReLex was 0.315 mm (0 to 1.131), while Femtolasik eyes had a mean decentration of 0.516mm (0.103 to 1.265) There were no complaints from any of the patients treated about monocular diplopia and/or other visual symptoms related to decentration.
The centration of the treatment zone as measured by Pentacam® is in favour of patient controlled fixation during the ReLEx procedure compared to active eye-tracker assisted Femtolasik surgery.
