Interventional case report of a 25-year old man with unilateral cataract in the right eye, without history of trauma or other identified cause, where an extended range of focus IOL (Symfony, Abbott) was implanted during cataract surgery.

Pre-operatively UDVA OD 0.3, CDVA OD was 0.5 with S+0.25=C-1.25*9. Five weeks postoperatively UDVA OD was 1.0, UNVA was J3 at 35 cm. His CNVA was J2 at 30 cm with S +1.0. He had no problems with intermediate activities and computer-work. He reported no dysphotopsias or halos in the distance. When performing near activities at 25 cm or looking at an illuminated screen at 30 cm he experiences some glare which is eliminated with reading glasses S+1.0. The patient was satisfied with the visual outcome.

An extended range of focus IOL may be a good option in unilateral cataract surgery in young, pre-presbyopic patients, who still have adequate accommodative ability in the contralateral eye.
