As most outsourcing companies have strict exclusion criteria they tend to take the less complicated cataracts away from training lists. There are subsequently fewer suitable cases for trainees to operate on.

Royal Gwent Hospital Theatres

Prospective audit from March 2016 to June 2016 assessing each cataract operated on a training list with a complexity score devised from a risk stratification proforma. The consultant was then asked if the cataract was our was not suitable for an st1-3.

67% of the cataracts were deemed not suitable for st1-3 by consultants. 73% of the cataracts had to be operated on by consultants. Only 27% of the cataracts were operated by trainees

Outsourcing has a significant impact on the case mix complexion of cataracts on training lists in the NHS. This poses a risk for cataract training as fewer straight forward cataracts are on NHS lists. Our study found only 27% of the cataracts on NHS training lists were actually operated on by ophthalmology trainees.
