Comparison of efficacy, safety and visual quality of SMILE and LASIK for myopia and myopic astigmatism

Abstract Details

Purpose:

To compare the efficacy, predictability and quality of vision after small incision lenticule extraction (SMILE) and laser insitu keratomileusis (LASIK) for myopia and myopic astigmatism.

Setting:

Centre for Sight, Banjara Hills, Hyderabad - India, a private tertiary care super specialty eye hospital.

Methods:

A retrospective comparative analysis of outcomes of SMILE (Group I) and LASIK (Group II) was done. Group. Group I consisted of 152 eyes of 76 patients with a mean spherical equivalent of -3.89 +/- 1.89 diopters and group II consisted of 204 eyes of 102 patients with a mean spherical equivalent of -3.26 +/- 1.25 diopters. Subjective visual complaints, refraction, visual acuity, mesopic visual acuity, low contrast visual acuity, corneal Q value, and higher order aberrations were measured preop and post operatively at 1 week, 1 month, 3 months and 6 months.

Results:

141 (92.76%) eyes in Gr. I and 185 (90.68%) eyes in Gr. II had a visual acuity of 20/20 at one month (p>0.5). 144 (94.7%) eyes in Gr.1 and 186 (91.1%) in Gr.II had a mesopic vision of 6/6 or better (p<.05). 138 (90.7%) eyes in Gr. I and 182 (89.2%) eyes in Gr. II had a low contrast acuity of 0.1 or better (p>0.5). 11 (14.4%) patients in Gr. I and 20 (19.6%) patients in Gr. II complained of glare at night (p<0.5). Gr. I (0.298+/-0.102um) had fewer aberrations(HOA) than Gr. II (0.410+/- 0.152um) (p<0.1).

Conclusions:

SMILE and LASIK are both safe and effective procedures for correction of myopia and astigmatism. However, SMILE induces lesser degree of higher order aberrations and offers a marginally better quality of vision as compared to LASIK.
