Importance of the anti-blockage technique in the phaco training of posterior polar cataracts

Attendee will learn the importance of the antiblockage technique using the ultrachopper and other maneuvers in dealing with posterior polar cataracts with soft or hard nuclei.

It has been demostrated by Vasavada et al and other studies that during hydrodissection the intraocular pressure raises the most. This technique helps to preserve the integrity of the posterior capsule in these challenging cases. In our training experience we use the Kitaro simulator system for the trainee in order to develop mental representations. Then in the patient, after doing the capsulorexhis, the surgeon makes a groove in the center of the nucleus with the ultrachopper and then split the two halves. Then we proceed to hydrodissection and the fluid runs through this space providing a better control of the posterior capsule integrity.Then we proceed with the maneuvers accordingly if it is a soft or hard posterior polar cataract.


In our experience, we have seen that our trainees can manage in a simple, safe, reproducible and effective procedure these cases. Also, since we are using this technique, the rate of posterior capsular rupture has decreased.
