Spontaneous migration of fluocinolone acetonide intravitreal implant in anterior chamber (AC)

Purpose:

To report a case of spontaneous migration of Fluocinolone acetonide itravitreal implant in anterior chamber (AC)

Setting:

In a Private Eye Hospital

Methods:

48 years lady presented with pain,redness,photophobia and decrease in vision in right eye for last 3 days duration.On examination she had circumcorneal congestion,sutures in inferotemporal sclera,marked descemets fold,aphakia with deficient posterior capsule and a small oval foreign body in AC inferiorly rubbing the endothelium.She gave history of having pars planitis for which she was Implanted Fluocinolone acetonide itravitreal implant 5 years back.She also underwent cataract surgery without intraocular lens two years after the implan.She also underwent paras plana vitrectomy for vitreous haemorrhage/drbris in same eye.She did not give any history of trauma. The implant was removed from AC by making a small incision in cornea under topical anaesthesia.The corneal endothelium was protected by viscodispersive viscoelastic.Patient was given hyperosmotic solution,steroid drop,antiglaucoma drops

Results:

Anterior chamber inflammation, corneal edema gradually resolved.She was found to have posterior capsular rupture with extension of anterior capsule tear.Fundus examination revealed evidence of healed pars planitis and macular pucker.Her visual acuity with aphakic correction was finger count at one meter.

Conclusions:

Spontaneous dislocation of intravitreal implant should be kept in mind FINANCIAL INTEREST: NONE
