Changes in retinal image in the accommodated eye after LASIK surgery: a simulation

Purpose:

To evaluate the quality of the retinal image in subjects who underwent LASIK surgery at different accommodative stimuli and to compare it with the quality of the retinal image before the surgery, by means of computer simulation.

Setting:

University of Valencia, Spain

Methods:

Computer simulations were performed for two scenarios: before and after the subjects were operated on. For this task, Zernike coefficients from the wavefront aberrations of 23 subjects before and after they underwent LASIK surgery were used. Then, two different accommodative demands (2 and 4 D), each one for two different pupil diameters (4 and 6 mm), were simulated modifying the spherical aberration according to previous studies. Finally, for each condition, the Point Spread Function (PSF), the Modulation Transfer Function (MTF) and examples of retinal images using targets with different spatial frequencies were computed and analyzed.

Results:

The different retinal image simulations showed how a certain target is seen by a subject before and after undergoing LASIK surgery, while the eye is accommodated. There were slight differences in the PSF, in the MTF, and in some of the retinal images between both scenarios: pre-LASIK and post-LASIK subjects. All these metrics changed also between different accommodative demands and pupil diameters.

Conclusions:

Corneal refractive surgery modifies the aberration pattern of the cornea. Slight differences in Zernike coefficients can be usually found between pre-LASIK and post-LASIK subjects.
