PresbyLASIK (Supracor corneal enhancement) after radial keratotomy: a case report with six months follow-up

Purpose:

To evaluate and present the efficiency and safety of Supracor procedure after radial Keratotomie (RK) in terms of quality of life improvement, visual acuity rehabilitation for distance, intermediate and near vision and to evaluate any changes regarding higher order aberrations (HOA´s) and the Q factor of the cornea.

Eye Clinic Orasis, Swiss Eye Research Foundation, Switzerland

Methods:

The patient underwent a bilateral same session Fs. LASIK with an asymmetric Supracor corneal enhancement for residual refraction errors and presbyopia treatment. Six months follow up with scheimpflug corneal topography, uncorrected distance visual acuity (UDVA), uncorrected intermediate visual acuity (UIVA), uncorrected near visual acuity (UNVA) and corrected distance visual acuity (CDVA) for both eyes was assessed. The patient was kindly asked to answer in a scale from 1 to 5 how she apprehended the changes in her life after treatment regarding the driving, PC- working and reading experience without spectacles and foreign body's sensations. One sample t-Test, paired sample t-Test and regression analysis was used for 12 samples. For one sample t-Test, the baseline value was used as the test value of comparison for the analysis. Importance was given for values less than p≤0.05.

Results:

The manifest refractive spherical equivalent (MRSE) shifted from hyperopia to Myopia on both eyes (mean -0,4 ± 1,16 Dpt.). UNVA and UIVA improved compared to baseline values (one sample t-Test, p<0,05) in both eyes. The UDVA remained stable until the 6th Mth (mean 0,48 ± 0,17). The CDVA improved compared to baseline and was better than UDVA (Paired sample, t-Test p<0,05). Corneal spherical aberrations and vertical Coma were reduced on both eyes but the means differed statistically on the right eye (One sample t-Test, p=0,05 respectively). A negative relation between higher order aberrations (HOA´s) and UDVA was shown (p<0,0124, R2=0,9754). Patient gave positive scores on the questions regarding her driving, reading and PC working experience after the treatment (One sample t-Tests, 3,16±0,9 p=0,03, 3,5±1,3 p=0,04 and 3,3±1,03 p=0,02 respectively) but gave negative score regarding the foreign body sensations. Corneal Q factor on 30° was reduced on both eyes but the means did not differed statistically between them.

Conclusions:

Supracor corneal enhancement after RK was proven in this case report to be of benefit for the patient. The quality of vision in all fields tested improved significantly and the patient stated spectacle free and an improvement in quality of life. Overall in this case Supracor enhancement was efficient and safe for the patient, however this is a case report and more studies need to surface with more patients in order to asses safety regarding presbylasik profiles treatments after Radial Keratotomy.
