Results of transparent lens phacoemulsification with IOL implantation in primary angle-closure glaucoma

Purpose:

Evaluate the hypotensive effect of the transparent lens phacoemulsification with IOL (intraocular lens) implantation in primary angle-closure glaucoma.

Setting:

Research was held in Kazakh Eye Institute from august 2013 till february 2014.

Methods:

We examined 35 patients (46 eyes) aged 40-60 years. All patients received antihypertensive local therapy. Subacute attacks, accompanied by periodic pain, rainbow circles were observed in 18 cases. Inclusion criteria were tonometry data (periodic increase in intraocular pressure (IOP)), visometry data (visual acuity above 0.7), biomicroscopy data (shallow anterior chamber, lens transparency), echobiometry data (anteroposterior dimension< 22,5 мм, anterior chamber< 2,50 мм, lens> 4,50 мм). As per optic disk and perimetry data developed stage is set on 23 eyes (50.0%) and advanced - on 23 eyes (50.0%). All patients underwent a surgery with IOL implantation phacoemulsification (IQ Alcon lens, USA) on the Infinity unit (Alcon, USA).

Results:

Operation and postoperative period was uneventful. IOP in the early postoperative period (up to 1 week) at all patients was compensated without local antihypertensive therapy. Later, IOP has increased on 30 eyes (65%), and therefore a spot antihypertensive therapy was appointed. In period of between 3 and 6 months glaucoma surgery was performed on 9 eyes (6 eyes - trabeculectomy, 3 - implantation of Ex-PRESS shunt). Subjectively, 85% of patients reported improvement: recurrent pain and a feeling of heaviness is stopped. Analysis of Heidelberg retinal tomography (HRT) results showed improvement in the dynamics of the morphometric parameters of the optic disc.

Conclusions:

In case of advanced stage of angle-closure glaucoma phacoemulsification with IOL implantation can compensate IOP in 30% of cases. In 30% of cases it is required a local antihypertensive therapy, 40% - hypotensive operation. Phacoemulsification with IOL implantation at an advanced stage helps to compensate for angle-closure glaucoma IOP in 50% of cases with application of local antihypertensive therapy and 50% - without its application.
