Clinical results of femtosecond laser-assisted cataract surgery with a 'glistening-free' intraocular lens implantation in patients with hard cataracts

Abstract Details



Purpose:

To evaluate the visual and refractive outcomes and IOL position after laser-assisted cataract surgery with a «glistening-free» EnVista intraocular lens implantation in patients with hard cataracts.

Setting:

Moscow, Eye Center 'East Sight Recovery'.

Methods:

: 52 eyes with hard cataracts ( nuclear dencity Grade 3 and Grade 4 or above) underwent femtosecond laser-assisted phacoemulsification with a «glistening-free» hydrophobic EnVista intraocular lens implantation. Preoperatively, all eyes had BCVA 20/100 or worse, 29% of eyes had BCVA 20/400 or worse. Optical coherence biometry (IOL MASTER, Carl Zeiss) was possible in 34,6% cases. Immersion ultrasound biometry was done in 65,4% cases. IOL calculation was performed with third-generation IOL formulas (SRK/T, Hoffer Q and Holladay). Femtosecond laser-assisted cataract surgery involved anterior capsulotomy and lens fragmentation. Phacoemulsification was performed through 2,2 mm clear cornea incision, using Stellaris PC phacomachine. Postoperatively, the effective phacoemulsification time (EPT), the average ultrasound power, refractive outcomes, best corrected visual acuity, IOL position, posterior capsule opacification (PCO) were estimated. The follow-up period was 9-15 months.

Results:

There were no intra-operative complications. A capsular tension ring was implanted in 5 cases. The EPT was 16,3±2,15 seconds in average, the average ultrasound power was 24%. On the first day 6 eyes ( 11,5%) had cornea edema. Target refraction (+0,5 D) was achieved in 81% cases. By the 3d month after operation 100% of eyes achieved a BCVA of 20/40. 82, 7 % of eyes had 20/25 or better BCVA. The right IOL position was observed in all cases. Posterior capsule opacification (PCO) was found in 9,6 % cases in 9-12 months after surgery. There were no cases of glistening in the follow-up period.

Conclusions:

The achieved results indicate that femtosecond laser-assisted cataract surgery with the «glistening-free» EnVista intraocular lens implantation is a safe and effective procedure in cases of hard cataracts. It allows to achieve a high level of target refraction and visual acuity. EnVista IOL reports a stable position, a low incidence of posterior capsule opacification and no glistening.
