Saccular endophthalmitis seven years after cataract extraction

To evaluate the course and visual outcome after the antibiotic irrigation of the capsular bag in a patient with saccular endophathalmitis

A 60-year-old man presented with gradual decline in visual acuity and blurred vision in his right eye over the past weeks. He had had an uncomplicated cataract surgery in seven years ago and three-piece IOL implantation in the capsular bag. His VA was 1/10 cc sph +0,25 cyl +0,25 x 85. He was diagnosed with saccular endophthalmitis and decided to undergo surgical vancomycin and ceftazidime antibiotic irrigation of the capsular bag and the anterior chamber. Sample was taken for culture and PCR analysis.


Capsular bag was cleaned significantly. VA improved to 9/10 cc sph +0,50. PCR was negative for Propionibacterium acne and fungi.


Antibiotic irrigation is the initial appropriate action for saccular endophthalmitis providing very good visual outcome in this case. FINANCIAL INTEREST: NONE
