Corneal stromal demarcation line following standard and high intensity collagen cross-linking determined with anterior segment optical coherence tomography

Purpose:

To compare the depth of the corneal stromal demarcation line using anterior segment optical coherence tomography (AS-OCT) after corneal collagen crosslinking (CXL) for two different treatment protocols (30 minutes with 3mW/cm2 versus 10 minutes with 9mW/cm2).

Setting:

Vardinoyiannion Eye Institute of Crete (VEIC), Faculty of Medicine, University of Crete, Heraklion, Crete, Greece

Methods:

Sixteen keratoconic patients (21 eyes) were enrolled. All patients underwent CXL using the same ultraviolet-A(UV-A) irradiation device (CCL-365, Peschke Meditrade GmbH, Huenenberg, Switzerland). Seven patients (9 eyes) were treated for 30 minutes with 3mW/cm2 according to the standard Dresden protocol (group 1) while 9 patients (12 eyes) were treated for 10 minutes (group 2) with 9mW/cm2 of UV-A irradiation intensity. One month postoperatively, corneal stromal demarcation line was measured by two independent observers using AS-OCT.

Results:

Mean corneal stromal demarcation line depth was 350.78 ± 49.34 μm (256.5 - 410 μm) for group 1 and 288.46 ± 42.37 μm (238.5 - 353.5 μm) for group 2. There was a statistically significant difference (t-test for unpaired data, p = 0.0058) in the corneal stromal demarcation line depth between the two groups; corneal stromal demarcation line depth in group 1 was significantly deeper than in group 2.

Conclusions:

Corneal stromal demarcation line depth after a 30-minute CXL treatment was significantly deeper than after a 10-minute CXL procedure with high intensity UV-A irradiation. FINANCIAL INTEREST: NONE
