Outcome of manual DSEK in failed graft

Abstract Details



Purpose:

To report the outcome of manual DSEK in failed primary graft.

Setting:

Tertiary care eye hospital in eastern India

Methods:

Manual DSEK was performed in 18 cases of failed primary graft between January to December 2013.The various indications of primary graft, graft clarity and visual outcome following DSEK as well as adverse events in the postoperative period were noted

Results:

14 eyes had clear graft at 1 year follow up with best corrected visual acuity improving upto 20/30 in 6 eyes. The mean lenticular thickness was 110.7 microns and an average anterior chamber depth of 2.84mm.Initial adverse events noted included graft dislocation(1 case),persistent interface fluid space(1 case) and 1 case of primary graft failure where re DSEK was performed. Late postoperative complications were noted in 4 eyes which demonstrated graft failure at final follow up with causes being rejection (1 case),infective keratitis(2 cases) and glaucoma(1case). In all eyes with graft failure, there was progressive stromal vascularization, which led to haziness in the graft necessitating full thickness penetrating keratoplasty.

Conclusions:

Manual DSEK is a viable alternative to regraft in cases with failed graft. FINANCIAL INTEREST: NONE
