Endothelial cell counts and final visual acuity in complex DSEK

To evaluate the visual outcomes and endothelial cell numbers in complex cases of DSEK

Retrospective analysis of 23 consecutive DSEK cases over 44 months that were deemed as complex due to pre-existing pathology. SPSS software was used. Patients were examined clinically at 1,3,6,9 and 12 months post-operatively where best corrected visual acuity (BCVA) and anterior segment optical coherence tomography (AS-OCT) were measured at these intervals. Patients underwent spectral microscopy at 12 months post-operatively.


23 cases were identified as complex. 30% were due to pre-existing corneal pathology, 30% of cases were due to aphakia, 40% of identified cases were due to graft detachments or previous DSEK graft failures. Mean donor endothelial cell count was 2570. Mean 12-month post-operative endothelial cell count was 1326 (reduction of 51.6%). Mean pre-operative BCVA was 1.48 LogMar. Mean post-operative BCVA at 12 months was 0.58 LogMar.


DSEK undertaken in this patient group lead to greatly improved BCVA while maintaining a sufficient number of endothelial cells. FINANCIAL INTEREST: NONE
