Low infusion pressure microincisional cataract surgery

Purpose:

Today most surgeons use traditional gravity based phacoemulsification systems to manatin anterior chamber stability throughout the surgery. Usual bottle heights are 90cm or even higher. This will result in high intraocular pressure in food positon 1 or during occlusion and high IOP change in unobstructed aspiration or situation of postocclusion break. This could have a significant impact to the eye and the outcome of cataract surgery. We therefore tested a new phacoemulsification sytem with active control of fluidics and IOP (Centurion Vision System, Alcon, USA).

Setting:

Aurelios Augenzentrum

Methods:

50 MICS with low infusion pressure and a preset of 20mmHg IOP were performed with the Centurion Vision System (Alcon, USA) were performed.

Results:

Anterior chamber stability at IOP of 20mmHg was very good. No complication occured. Esp. no post-occlusion break was recognized. Settings were optimized to this new technology.

Conclusions:

Active control of fluidics during phacoemulsification seems to be superior to traditional gravity based infusion systems and allow safe cataract surgery at very low IOP levels.
