Abstract Details

Purpose:

Evaluation of the distance of a phakic IOL (ICL) to the crystalline lens (vaulting) by intraoperative optical coherence tomography (OCT).

Methods:

All examinations were taken with the Rescan 700 (Carl Zeiss Meditec). To date, due to technical reasons a direct measurement of structures with the intraoperative OCT is not possible. It only allows visualization of structures without giving a concrete distance value. Accordingly, the central thickness of the ICL (150 μm according to the manufacturer) was used as a reference plane. Measurements were taken directly after the implantation of the ICL in the anterior chamber filled with viscoelastic in the vertical position, after rotation in the horizontal position and after removal of the viscoelastic. Results were compared with the postoperative vaulting.

Results:

Good visualization of the ICL and its position by intraoperative OCT could be demonstrated. The indirect evaluation of the vaulting between the ICL and the crystalline lens showed a good consistency to the postoperative measurements.

Conclusions:

The intraoperative assessment of the vaulting between the ICL and the crystalline lens is possible and allows for the first time to estimate the vaulting and the positioning of the ICL during surgery. This technique can help to prevent vault related complications like pupillary block due to a hypervault in an ICL to large in diamter or contact with the crystalline lens due to an ICL to small in diameter.
