Abstract Details

Purpose:

To compare the cytokine/chemokine expression profile in the aqueous humour (AH) of patients undergoing femtosecond laser-assisted cataract surgery (FLACS) vs. conventional phacoemulsification accompanied by preoperative instillation of bromfenac 0.09% or indomethacin 0.01%

Setting:

S. Fyodorov Eye Microsurgery State Institution, Moscow, Russia Scientific Centre of Children Health, Russian Federation Center for Eye Research, Department of Ophthalmology, Oslo University Hospital and University of Oslo, Norway

Methods:

bromfenac (Group 1; 14 eyes) or indomethacin (Group 2; 12 eyes), as well as from control patients undergoing conventional phacoemulsification with the same instillations (Group 3; 12 eyes, and Group 4; 17 eyes, respectively). Measurements of the interleukin (IL)-1β, IL-2, IL-4, IL-5, IL-6, IL-8, IL-10, IL-12p70, interferon (IFN)-γ, tumour necrosis factor (TNF)-α, TNF-β concentrations in AH were performed using FlowCytomixPro (eBioscience). The pupillary dynamics were estimated by recording the intraoperative video and two perpendicular values of pupil size were measured in the Rulers (Omnidea)

Results:

Group 2 showed higher aqueous humor levels of IL-2 (p<.02), IL-4 (p>.02), IL-5 (p<.02), IL-6 (p<.02), IL-8 (p<.02), TNF-α (p<.02), TNF-β (p>.02), IFN-γ (p<.02), IL-12p70 (p>.02), while no difference in IL-10 concentrations (p>0.02) was observed when compared to Group 1. Groups 3 and 4 showed lower concentrations of all cytokines (IL-2 (p>.02), IL-4 (p>.02), IL-5 (p<.02), IL-6 (p<.02), IL-8 (p>.02), TNF-α (p<.02), TNF-β (p>.02), IFN-γ (p>.02), IL-12p70 (p<.02)) when compared to Group 1 and 2, respectively. Pupil size appeared to be more stable in Group 1, while no occurrence of significant pupillary miosis could be registered

Conclusions:

The results suggest that the inflammatory reaction in FLACS may be more activated with indomethacin 0.01% preoperative instillations than with bromfenac 0.09%, and it can lead to higher levels of pupillary constriction compared to FLACS instilled with bromfenac 0.09% preoperatively. Overall, lower level of infammatory response could be registered in the group without femtosecond laser-assistance
