Trifocal IOL implantation in refractive lens exchange after PresbyLASIK: report of a case

Abstract Details

Purpose:

In eyes after presbylasik presenting for lens surgery it is unclear how to calculate the IOL power, and if multifocal IOLs can be an option. We report a case of refractive lens exchange with trifocal IOL implant that had good outcome.

Setting:

Vista Vision Laser Centre, Verona, Italy

Methods:

A male patient aged 56 with +1.5 D of hyperopia presented for refractive surgery in 2014, asking both for distance and for near vision correction. He underwent bilateral femto-presbylasik with the Supracor technique. In the post-operative he complained of poor near vision, despite the good measured uncorrected visual acuity: 0.0 logMAR for distance and 0.1 logMAR for near. In 2016 he asked for refractive lens exchange, and underwent FLACS with the implantation of a trifocal intraocular lens in both eyes.

Results:

IOL power calculation was performed with the ASCRS on-line calculator, computing +20.0 IOL power bilaterally. Surgery was uneventful. One year after surgery uncorrected vision is is 0.0 logMAR for distance, and 0.06 logMAR for near. The aberration profile of the eyes shows no increase in high order aberration as compared with trifocal IOLs. The patient is satisfied with the outcome, and reports good no glare and no dysphotopsia.

Conclusions:

The implant of a trifocal IOL after presbylasik was associated with good outcome in this patient, both for the precision of power calculation and for the quantity and quality of vision.
