Evaluation of efficacy and safety of pneumatic trabeculoplasty in glaucoma and ocular hypertension
PURPOSE: The aim of this study was to determine the efficacy and the safety of PNT alone and in combination with topical monotherapy to lower IOP in patients with primary open angle glaucoma (POAG) or ocular hypertension (OH). SETTING: Department of Ophthalmology, Ospedali Civili Riuniti di Sciacca, Sciacca, Italy. METHODS: A total of 40 subjects (79 eyes) with POAG or OH were enrolled in a prospective trial to determine the IOP lowering effect of PNT. Prior to the start of study all subjects underwent ophthalmologic examinations, IOP by applanation tonometry was evaluated three times on different days and a visual field was performed. PNT was performed at days 0 and 7. A regular follow-up was carried out for 6 months every 4 weeks, and IOP was measured at different time points. Of the 40 patients enrolled, 37 completed the study. RESULTS: The pre-treatment mean IOP was 22.5+/-1.5 mmHg. The differences between the post-treatment mean IOP and the baseline mean IOP were statistically significant (p<0.05) at all time points. The mean IOP reductions from baseline mean IOP were seen on first day (-20%), 1 week, 4 weeks (-19.6%), 8 weeks (-17.2%), 12 weeks (-16.5%), 16 weeks (-14.5%), and 24 weeks (-14.6%). No significative difference were found between patients with topical treatment and without. Conjunctival hyperemia occurred in all eyes and conjunctival haemorrhage in 4. One patient had hypertension in one eye and no significant reduction of the IOP during the follow-up. CONCLUSIONS: This study showed a good IOP lowering effect on glaucoma and hypertensive patients. However a much larger cohort study over a considerable number of years will eventually demonstrate its safety and would help to identify risk factors that may lead to treatment failure and duration of the effect.
