Long-term outcomes of corneal modulation with excimer laser for simultaneous correction of presbyopia and ametropia

Abstract Details

Purpose:

To investigate the long term outcomes of simultaneous correction of presbyopia and ametropia by a bi-aspheric cornea modulation technique based on the creation of a central area hyper-positive for near vision and leaving the pericentral cornea for distant vision.

Setting:

VeniVidi Eye Hospital - Istanbul

Methods:

822 presbyopic patients (1644 eyes) (52±4 years, range 41-59) with distance refraction spherical equivalent 2.05 D ± 2.01 D in dominant eyes (DE) and 0.98 D ± 2.6 D in nondominant eyes (NDE) and astigmatism up to 3 D with presbyopic add +1.25 D to +3.00 D underwent LASIK were assessed up to 36 months postoperatively. All eyes underwent the corneal treatment using the PresbyMAX® software, delivering a bi-aspheric multifocal ablation profile developed by SCHWIND eye-tech-solutions (Kleinostheim, Germany). All flaps were created by the pendular SCHWIND eye-tech-solutions microkeratome (Kleinostheim, Germany).

Results:

The mean preoperative uncorrected distant visual acuity (UDVA) of DE and NDE were 0.31 ± 0.82 logMAR and 0.34 ± 0.82 logMAR respectively. The mean postoperative UDVA increased to 0.04 ± 1.22 logMAR and 0.17 ± 0.82 logMAR in DE and NDE respectively (p 1.2<0.001). The mean spherical equivalent of refractive errors decreased to -0.32 D ± 0.32 D and -0.59 D ± 0.39 D in DE and NDE respectively (p 3.4<0.001). Post operative uncorrected near visual acuity of DE and NDE were 0.45 ± 0.12 logRAD and 0.43 ± 0.10 logRAD respectively.

Conclusions:

In presbyopic patients without symptomatic cataracts, but refractive errors, PresbyMAX® will decrease the presbyopic symptoms and correct distance refraction in the same treatment session, offering spectacle-free vision in daily life in most of the treated patients.
