Impact of the near addition on the optical quality of diffractive multifocal intraocular lenses: an analysis at the optical bench

Abstract Details

Purpose:

To analyze the optical quality of diffractive multifocal intraocular lenses (IOLs) with different near additions in a laboratory study.

Setting:

David J Apple International Laboratory for Ocular Pathology, University Eye Hospital Heidelberg, Heidelberg, Germany

Methods:

We analyzed the AcrySofIQ ReSTOR2.5 (Alcon, USA), the AcrySofIQ ReSTOR3.0 (Alcon, USA) as well as the TECNIS ZMA00 and ZMB00 (Abbott Medical Optics, USA) with a power of 21D for the distance using the OptiSpheric optical bench (Trioptics, Germany). The lenses have a similar optical design but different near additions: The ReSTOR2.5 has a near addition of +2.5D, the ReSTOR3.0 has a near addition of +3.0D and both TECNIS IOLs have a near addition of +4.0D. We evaluated the modulation transfer function area (MTFA) and the Strehl ratio using a 3 mm (photopic) and 4.5 mm (mesopic) aperture, respectively.

Results:

The MTFA at the far focus (ReSTOR2.5/ReSTOR3.0/TECNIS) was 47.85/42.06/33.57 (3 mm aperture) and 26.06/33.08/14.83 (4.5 mm aperture). The MTFA was 11.05/9.54/8.60 (3 mm aperture) and 4.83/4.39/5.33 (4.5 mm aperture) for intermediate distances. MTFA at the near focus was 20.11/29.50/33.39 (3 mm aperture) and 7.27/12.54/24.76 (4.5 mm aperture). The Strehl ratio was 0.48/0.42/0.29 (3 mm aperture) and 0.26/0.33/0.13 (4.5 mm aperture) at the far focus. At the intermediate focus, the Strehl ratio was 0.11/0.10/0.09 (3 mm aperture) and 0.05/0.05/0.06 (4.5 mm aperture). At the near focus, the Strehl ratio was 0.20/0.30/0.34 (3 mm aperture) and 0.07/0.13/0.25 (4.5 mm aperture).

Conclusions:

At the near focus, the ZMA00 and ZMB00 performed the best, while the ReSTOR 2.5 performed the worst. At the intermediate distance, all IOLs achieved similar results. At the far focus, the ReSTOR 2.5 performed the best. Surgeons can choose the right IOL for the patients’ individual needs, because a broad range of near additions in diffractive multifocal IOLs is available these days.
