OCT analysis for scleral lens fitting on unusual topographic patients

Abstract Details


We analysis the usability of anterior segment OCT (RTVue) for the fitting, safety and comfortableness of scleral contact lens.


Total 40 eyes of 37 patients of which keratoconus and unusual topographic distortions caused by surgery or trauma were fitted with Blanchard Onefit scleral lens.


We evaluated AS-OCT on central apical clearance and 4 quadrants limbal clearances at 30 minutes after first and second trial lens fitting, and final own lens fitting. Also we checked slit lamp evaluation and subjective symptoms.


Apical clearance were 254+- 89.72, 237+- 66.12 and 211+-47.14um at first, second and final fitting. Second. Vertical quadrant limbal clearance were 179+-82.75, 137+-78.94 and 101.55+- 94.66um at first, second and final fitting. Horizontal limbal clearance were 275+-141.48 220+-98.11 and 150.41+- 101.74um 66um at first, second and final fitting. Three eyes had conjunctival injection and pain due to too shallow limbal clearances (tight peripheral curve).


AS-OCT is a very valuable evaluation tool for the highly customizable lens fitting and problem solving, especially unusual topographic distortions. Limbal vault which is could not be measured with slit lamp is important in these special cases.
