Evaluation of changes in corneal biomechanical properties at different levels of diabetic retinopathy

Abstract Details


To evaluate the corneal biomechanical properties due to the glycosylated hemoglobin (HbA1C) levels using the ocular response analyzer (ORA) in patients with type 2 diabetes mellitus (DM).

ORA values were obtained from one randomly chosen eye of 101 subjects with type 2 DM and 40 healthy control subjects with similar age and sex. Subjects were divided into three groups: Group 1: healthy control subjects; Group 2: DM patients with HbA1C <7%; and Group 3: DM patients with HbA1C ≥7%. Corneal biomechanical parameters: corneal hysteresis (CH), corneal resistance factor (CRF), Goldmann-correlated intraocular pressure (IOPg), and corneal-compensated intraocular pressure (IOPcc). Ultrasound pachymetry was used for measurement of central corneal thickness (CCT).


CRF was found to be significantly lower in Group 1 and 2 when compared with Group 3 (P values for CRF respectively; Groups 1 and 2=0.63, Groups 1 and 3, and Groups 2 and 3, = 0.01). There was no significant difference in terms of CH. CCT was significantly different between Groups 1 and 3 and Groups 2 and 3 (P = 0.01) but was insignificant between Groups 1 and 2. Both IOPg and IOPcc was not statistically different between Groups 1 and 2, and Groups 2 and 3, but was significantly different between Groups 1 and 3 (P=0.038).


In poorly controlled diabetics, CRF and CCT are significantly higher compared with those of the healthy subjects and patients with well-controlled diabetes. These values should be taken into account when measuring IOP values in pacients with diabetes mellitus.
