To report a case of modified Deep Anterior Lamellar Keratoplasty (DALK) for the management of corneal perforation.

A 76-year-old man suffering from corneal perforation in his left eye (LE) was referred to our clinic. The patient had undergone two attempts of conjunctival auto-grafting. Best corrected visual acuity (BCVA) of his LE was hand movement (HM), while slit- lamp examination revealed a central corneal perforation sealed by iris prolapse. The patient underwent urgent modified DALK with a partial thickness corneal graft which was provided by the AHEPA University Hospital Eye Bank of Thessaloniki.


Modified DALK led to anatomical restoration of the globe, while VA improved to counting fingers (CF). The cornea remained clear, apart from the initial site of perforation, with a low risk of graft rejection, since the host endothelium was left in place.


Modified DALK is a safe and efficient tectonic method for the management of corneal perforation.
