Abstract Details

To evaluate Osteo-Odonto-Keratoprosthesis (OOKP) as a treatment option for visual rehabilitation in corneal blindness in severe cases of Graft versus Host Disease (GvHD).

University Eye Hospital Basel, Primary Care Center

Presentation of a 60 year old man that developed severe ocular GvHD with bilateral corneal perforation after stem cell transplantation for multiple myeloma requiring multiple corneal grafts. Ulcerations after penetrating keratoplasty (PKP) were treated with botox ptosis and tarsorrhaphy on the right eye, while repeated allograft rejections with severe ulcerations were controlled by amniotic membrane application on the left eye. Due to bilateral corneal blindness OOKP was performed in the right eye. Recurring mucosal meltings were treated with modified Hughes plastics.

After challenging covering of the prosthesis in the beginning, surface problems were stabilized and visual acuity was 0.3 only four weeks after surgery.

OOKP is a safe and feasible treatment option for corneal blindness in GvHD patients. Although wound healing, vascularization and mucosal melting were difficult, no alternative could be used in the dry conditions of GvHD.
