Outcomes of Descemet's striping automated endothelial keratoplasty (DSAEK) for failed penetrating keratoplasty in Argentina

Abstract Details

Purpose:

To evaluate graft survival and visual outcomes in patients who underwent endothelial keratoplasty (DSEK) for failed penetrating keratoplasty (fPK) and compare outcomes with patients with DSAEK for Fuchs dystrophy (FD) and pseudophakic bullous keratopathy (PBK) . Number of patients needing rebubbling, graft failure and endothelial cell count loss were assessed as well.

Setting:

Cornea Service. Department of Ophthalmology, Italian Hospital in Buenos Aires, Argentina.

Methods:

Visual outcomes of the 67 cases of DSEK performed between January 2010 and November 2013 were analyzed retrospectively. Donor corneal- scleral rims were dissected with a microkeratome and were cut with a trephine. All pacientes had Descemet's stripping. The graft was inserted using Busin funnel glide (Moria, Anthony, France).

Results:

Mean patient age was 74.98 SD 4,37 years. 10 (14.9%) were treated for fPK, 33 eyes (49.25%) for PBK, and 24 (35.82%) for FD. Mean follow up was 7.6 SD 0.97 months. BCVA improvement was 0.99 (P=0.11), 2.44 (P=0.02) and 2.36 (P=0.00) lines for fPK, PBK and FD respectively, overall improvement was 2.28 lines (P=0.00). Rebubbling was needed in 19,40 % (n= 13); the failed PK 40%, followed by PBK 24%and Fuchs 16% Endothelial cell loss was 56%, 42% and 38% for fPK, PBK and FD respectively, overall cell loss was 43 %. Graft failure was higher in the failed PK subgroup (n= 5, 50%), followed by FD n=1, 4.16%) and PBK (n=1, 3%).

Conclusions:

DSEK provides better visual outcomes in the cases with PBK or FD than cases with fPK . Percentage of rebubbling and primary and late graft failure were higher in the study group compared to the others.
