To assess the effectiveness of tacrolimus in treating severe dry eye syndrome in a case of Naxos disease.


Ophthalmology department. Reina Sofia University Hospital, Córdoba, Spain.


Case report


7-year-old male diagnosed of Naxos disease transferred to our Center for assessment of severe dry eye. Parents concern intense photophobia, watery eyes and eye redness. Visual acuity (VA) was 0.1 both eyes (BE). Mild ectropion on the lower eyelid BE, conjunctival hyperemia with signs of squamous metaplasia and paracentral leucoma was shown at Biomicroscopy. Tear meniscus was less than 0.4 mm and BUT of 1-2 seconds. Without retinal changes. After several treatments, the patient was treated with Tacrolimus 0.03% ophthalmic ointment twice a day. 8 months later he showed a subjective clinical recovery, improvement in visual acuity measured 0.5-0.6 and a meniscus tear increased.


Topical treatment with tacrolimus 0.03% may be an alternative in cases of severe dry eye that have not responded to standard treatments. Further study is warranted to assess the long-term efficacy and safety.
