Abstract Details


To present the management of cataract in a case with megalocornea, zonular dialysis and posterior crocodile shagreen.


Department of Ophthalmology Selcuk University


A 51 years old patient with extremely wide corneas and deep anterior chamber (5.47mm OD, 5.93 mm OS) was admitted to our department with diagnosis of cataract in right eye. There was nuclear sclerosis and phacodonesis in the right eye with megalocornea. The corrected visual acuity was 0.05 as measured with Snellen chart.


Despite the extreme deep anterior chamber and presence of loose zonules, phacoemulsification surgery was uneventfully performed with implantation of a 13 mm wide capsular tension ring and three piece IOL. During the postoperative follow up visual acuity improved up to 0.7 as measured with Snellen chart.


Phacoemulsification in cases with megalocornea needs careful surgery due to extreme deep anterior chamber, capsular tension rings should be use to achieve safe and stabile implantation of IOL.
