Comparison of iris fixation versus sutureless scleral fixation of intraocular lens in aphakia

Abstract Details

Purpose:

to analyse whether retropupillary fixation of rigid iris claw lens is a viable option to correct aphakia in absence of adequate capsular support or a sutureless scleral fixation is a better option

Setting:

Retrospective analysis in Tertiary care centre

Methods:

Those Patients who had aphakia with inadequate capsular support either as a consequence of intra operative complications or dislocated cataractous lens or IOL or those who presented in the General Out-Patient Department of a tertiary care eye hospital for secondary implantation of IOL were analysed.

Results:

50 cases underwent sutureless scleral fixation of IOL(SFIOL) and 64 cases of Iris fixation of IOL(IFIOL). The 64% cases underwent SFIOL as a secondary procedure in SFIOL group whereas 71.87% cases underwent a primary procedure in the IFIOL group.Visual acuity in the range of 6/6 – 6/12 at six months postoperative was 57.14 % and with visual acuity in the range of 6/18-6/60 were 42.86% in the IFIOL group whereas visual acuity in SFIOL group was 88% in the range of 6/6 to 6/12 and about 10% of cases had visual acuity in the range of 6/18 to 6/60 at six months post-operativeand the The mean endothelial loss was 11.78% in IFIOL group and 5.5% in SFIOL group.

Conclusions:

SFIOL procedure can be a recommended procedure to be learnt by the anterior segment surgeons.For IOL implantation in cases with inadequate capsular support with only anterior vitrectomy being done & not requiring core vitrectomy IFIOL can be done though it needs to be further researched. In cases where nucleus was dropped SFIOL was done than IFIOL.
