Abstract Details

Purpose:

To compare the efficacy and the visual outcomes of toric intraocular lens (IOL) implantation, limbal relaxing incisions (LRI) and opposite clear corneal incision (OCCI) for management of coexisting astigmatism on cataract surgery.

Setting:

Cornea and refractive surgery subspecialty.

Methods:

This prospective comparative study included 45 eyes of 45 patients with visually significant cataract and coexisting corneal astigmatism were randomized to undergo phacoemulsification with either toric IOL or LRI or OCCI. The main outcome measures were postoperative 3-month uncorrected visual acuity (UCVA), cylindrical values, and the surgical induced astigmatism.

Results:

At 3 month, Uncorrected distance visual acuity (UDVA) was statistically significantly better in all 3 groups according to baseline measurements (p<0.0001, p<0.0001, p<0.0001). The mean cylindrical values were decreased from 2.62 ±0.87D for LRI, 2.72 ±1.31D for OCCI and 2.69 ±0.69D for toric IOL group to 1.12 ±0.54D for LRI, 1.31 ±1.28D for OCCI, 0.69 ±0.47D for toric IOL group ( p<0.0001, p<0.0001, p<0.0001). Surgical induced astigmatisms for the correction of refractive cylinder were 2.16 ±0.32D for LRI, 1.87 ±0.84D for OCCI, 2.64 ±0.81D for toric IOL group. Comparisons of the groups reveal significant difference only with toric IOL and OCCI group. (p=0.017)

Conclusions:

All LRI, OCCI and toric IOL implantation groups were effective on surgical correction of refractive astigmatism. In this comparative study the toric IOL implantation was found more effective in correcting corneal astigmatism.
