Surgical outcome of phacoemulsification with Malyugin ring in comparison with iris hooks in eyes with small pupil

Abstract Details


To compare and evaluate the operative outcomes of patients using phacoemulsification with malyugin ring and iris hooks in eyes with small pupil.

A total of 84 eyes of 63 patients with small pupil less than 3.5mm were operated with temporal clear corneal phacoemulsification in a period of three years between 1st January 2012 to 1st January 2015.The patients were divided equally into two groups with 42 eyes each in both the groups.Group 1 consisted of patients whose pupils were enlarged using Malyugin ring while in Group 2 the pupil was widened using 4 iris hooks. All cases were performed by a single surgeon under peribulbar anaesthesia using the small pupil management devices.


Intraoperative complications were negligible with few incidents of anterior chamber instability,descemets stripping and uncontrolled rhexis formation in both the groups.However linear mark imprints at the pupillary margin were seen more commonly in Group 2 as compared to Group 1 (8 cases in group 1 and 22 cases in group 2). Post operative complications included increased intraocular pressure, cystoid macular oedema and posterior capsular opacification which were very few in both the groups and statistically not significant.Mean BCVA in Group 1 recorded was 6/9 or better while in Group 2 it was 6/12 or better .


Adequate exposure of pupillary area using pupil expanding devices helps overcome small pupil management.Malyugin ring makes phacoemulsification more safe and effective in these cases with lesser complication rates and better visual outcomes as compared to Iris hooks.
