Abstract Details

Purpose:

Presentation of the innovative method of ammetropia correction for teenager patient by implanting presbyopic phakic IOL EyePCL (EyeolUK) in the sulcus as an “add on” implant to correct both distance and near refraction.

Setting:

Gemini Eye Clinic, Zlin, Czech Republic

Methods:

17 year old girl who was operated for congenital cataract at the age of 6 months with monofocal PMMA in the bag implantation with optic capture came with a request for surgical correction of her myopia with astigmatism. Her corneas were asymmetric with significant inferior bulging considered a contraindication for corneal laser refractive surgery. We suggested an implantation of presbyopic EyePCL phakic IOL. This implant number one is not intended for pseudophakic eyes and number two is intended for presbyopic patients as its diffractive structures are intended to correct presbyopia. After posterior synechiolysis the implantation was performed without complication.

Results:

Before the surgery refraction right eye: -4,0 -2,0 ax 130 and left eye -7,25 -2,0 ax 120. CDVA each eye 0,5. 6 months after the surgery UDVA each eye 0,5. UNVA OD J11, OS J2. CNVA ODS J1. Refraction near OD +1,0 +1,5 ax 30 and OS +1,25. She had difficulties with reading and writing at school for the first month after surgery. But her uncorrected near vision gradually improved and she is spectacle independent and very happy. Her OD UNVA is significantly worse than OS UNVA perhaps because of higher corneal asymmetry.

Conclusions:

The unusual combination of PMMA and diffractive presbyopic phakic implants is capable to achieve spectacle independence for both distance and near.
