Abstract Details

Purpose:

To compare polymerase chain reaction (PCR) with culture and smear in suspected microbial keratitis and endophthalmitis for detection and identification of pathogens between January –December 2015

Setting:

Tertiary Eye Care Centre of Eastern India

Methods:

Corneal scrapings from 22 patients with suspected infectious keratitis and vitreous sample of 34 endophthalmitis patients were taken for routine Gram's stain, potassium hydroxide (KOH) wet mount, culture sensitivity and PCR. The result of the PCR was compared with conventional culture and Gram staining method for both the group of patients.

Results:

In corneal scraping, smear positive were 19 cases (86%), culture positive were 13 cases (59%) and PCR positive were 20 cases (91%), whereas vitreous smear were 13 cases (38%), culture were 9 cases (26%) and PCR were 29 cases (85%) positive. Among corneal scraping group, PCR for fungus were 15 positive cases (75%) and culture positive fungus were 6 cases (46 %). Incidences of bacterial PCR were 5 cases (25%) and bacterial culture positive were 7 cases, (54 %). While, in vitreous sample group, PCR for bacteria were 22 cases (76%), and culture for fungi were 2 cases (22%) positive.

Conclusions:

DNA amplification with universal primers is a promising diagnostic tool in cases of ocular infection where routine laboratory culture has failed to identify the pathogen. PCR may be performed in cases where the results of vitreous stains are negative, without waiting for the results of the laboratory culture.
