Abstract Details

Purpose:

To evaluate the impact of Intra Corneal Ring Segments (ICRS - INTACS) implantation on quality of life of patients with severe keratoconus using Vision related Quality of Life (VrQOL) questionnaire.

Setting:

Ojas Laser Surgery Centre, Bandra, Mumbai, India. Single Centre Single Surgeon.

Methods:

Retrospective non randomized case series in 32 eyes of 16 patients (Age 25.94 + 8.93); Male:Female ratio 13:3 done. Patients with severe keratoconus who underwent bilateral INTACS implantation who completed minimum one year follow-up were included. Postoperatively 1 week, 1 month, 3 months, 6 months, end of one year and subsequently every year; UnCorrected Visual Acuity (UCVA), Best Corrected Visual Acuity (BCVA), Manifest Refraction (MR) and Topographic Indices (TI) using pentacam; VrQOL questionnaire (pre and post wearing best correction for minimum one month at end of 1 year) were noted. Double segment INTACS using femtosecond done. No complications were noted.

Results:

There was significant improvement in all parameters (UCVA, BCVA, MR, Keratometry and TI). Binocular best corrected visual acuity improved which was statistically significant +0.5 logMAR before to +0.2 logMAR after surgery with significant decrease in keratometric (Rmin) from 5.97 + 0.67 to 6.75 + 0.66 postop and topographic indices (IHA) preop 34.56 + 13.74 to postop 14.52 + 11.45 (Paired t and Wilcoxon). Post INTACS, quantity of vision (far and near), appearance and satisfaction with correction was good; 12.5% of patients required no correction, 81.25% (13) patients were happy with correction and 6.25% were not satisfied.

Conclusions:

INTACS significantly improved quantity and quality of vision with a positive impact on quality of life. It can be used for bilateral implantation; however a large case series is required.
