Abstract Details

Purpose:

To report the preoperative astigmatism evaluation of a bilateral pellucid marginal degeneration (PMD) with cataract that was successfully treated with implantation of a customized high-power IOL (AT TORBI 709 M)

Setting:

Methods:

PMD stability was bilaterally evaluated during 2 years by differential maps on Scheimpflug imaging and topographic analysis (OPD). MPdistance was in right eye 0.14 and left eye 0.19, the preoperative corrected distance visual acuity (CDVA) was on both eyes 5/10 with 13.19 diopters (D) right eye and 11.51 diopters (D) left eye of keratometric astigmatism on OPD Topographer. Total and HOA corneal Wave-Front were examined, ACD (antherior chamber depth) measured and ELP (effective lens position) calculated.

Results:

After implantation of an IOL in right eye with +13.50D +9.50D 5° and in left eye with +11.50D +9.00D 2° the CDVA was in right eye 9/10 -3.50cyl ax110° and left eye 9/0 -3.00cyl ax 100°. No surgical complications problems occurred, and postoperative internal aberrometry showed right orientation and astigmatism compensation of IOL.

Conclusions:

Toric high-power customized IOL can be a good choice in cataract surgery with pellucid marginal degeneration; stability of ectasia, regularity of astigmatism and low rate of aberrations are mandatory conditions to obtain a good visual outcome.
