Abstract Details

Purpose:

To analyse visual acuity and contrast sensitivity under different lighting and glare conditions after bilateral cataract surgery with implantation of Tecnis ZCB00 monofocal intraocular lens (IOL) and Tecnis ZMB00 multifocal IOL.

Setting:

Miguel Servet University Hospital, Zaragoza, Spain.

Methods:

Prospective study with one hundred eyes of sixty patients with implantation of an IOL after bilateral cataract phacoemulsification surgery. Thirty six eyes were implanted a Tecnis ZCB00 monofocal IOL and eighty eyes were implanted a Tecnis ZMB00 multifocal IOL. Patients met inclusion criteria and had a deep ophthalmic preoperative examination. After surgery, visual acuity and contrast sensitivity were examined under photopic lighting conditions with and without glare, and under mesopic conditions with and without glare.

Results:

Both groups improved their distance visual acuity under photopic conditions with and without glare but there were no differences between them. Under mesopic conditions, the monofocal group achieved a better distance binocular visual acuity without optical correction and a better distance monocular visual acuity with correction. This group also achieved a better visual acuity without optical correction under glare and mesopic conditions both monocularly and binocularly. CSV-1000E test for contrast sensitivity showed better outcomes for the monofocal group under different lighting and glare conditions. This difference was bigger at medium and high frequencies under mesopic conditions, but worsened with glare.

Conclusions:

Tecnis ZMB00 multifocal IOL achieve an acceptable distance vision and contrast sensitivity, similar to the Tecnis ZCB00 monofocal IOL under photopic lighting conditions. However, under low illumination conditions, contrast sensitivity is worse with Tecnis ZMB00 multifocal IOL. Nevertheless, these outcomes are within the age-adjusted normal range. Monocular visual acuity is lower in the multifocal group under low illumination conditions and especially with glare but it seems to be no difference when binocularly and without refractive deffect.
