Abstract Details

Purpose:

To estimate the results of IOL power calculation using Hoffer Q, BESSt 1.0, ASCRS IOL calculator (http://iolcalc.org) formulas in patients after radial keratotomy with Scheimpflug principle (Oculus Pentacam). To develop recommendations on preoperative investigation for more accurate calculation of IOL power.

Setting:

IRTC Eye Microsurgery Ekaterinburg Center, Ekaterinburg, Russia

Methods:

The investigated group included 95 patients (106 eyes) who had previously undergone radial keratotomy. Mean age of the patients was 57.0 ± 10.0 years. 57 males and 38 females. Time after radial keratotomy was 21.5 years. Retrospective analysis of patient’s records after lens surgery with IOL implantation was performed. Based on manifest refraction after surgery re-calculation of implanted IOL power for emitropiya performed. Then IOL power calculation performed based on preoperative investigation data using Hoffer Q, BESSt 1.0, and ASСRS IOL calculator formulas. The results calculated with the formulas compared with the refraction of the “required” IOL for emitropiya.

Results:

Analyzed groups Group 1: “pseudonormal” cornea (with “pseudonormal” distribution of corneal refraction in the central optical zone) - 23 eyes. Group 2: “pathologic” cornea (with “pathological” distribution of corneal refraction in the central optical zone) - 83 eyes. The results of IOL power calculation were analyzed and compared in these two groups.

Conclusions:

In patients with cataract after previous radial keratotomy it is necessary to investigate corneal refraction with Pentacam in 4maps Refractive, Holladay EKR modes. In patients with “pseudonormal” distribution of corneal refraction it is preferable to calculate IOL power using ASCRS IOL calculator formula. In patients with “pathologic” cornea IOL power is calculated individually. The results are unpredictable. The smallest error is achieved with ASCRS IOL calculator formula and Hoffer Q formula using refractive value of 1.0 mm zone.
