Abstract Details

Purpose:

• The different modes of femtosecond laser nuclear division (LenSx Alcon) require evaluation and comparison with Micro Incision Cataract Surgery technique (Alcon Centurion Vision System). • We propose to compare Cumulative Dissipated Energy (CDE) in dense cataracts and post operative Corneal Volume and Corneal Endothelial Count among 3 groups i.e. Femto Laser Grid pattern (Group 1) and Femto Chop Cylinder pattern of nucleus division (Group 2) with Micro Incision Cataract Surgery

Setting:

Prospective & non-randomized clinical trial of 120 eyes (120 patients) was done at a tertiary Care Eye Hospital, India.

Methods:

In Group 1, 40 eyes (40 patients) underwent cataract surgery with Femto Laser assisted (Alcon LenSx) 400 microns Grid pattern of pretreatment in cataract. In Group 2, 40 eyes (40 patients) Femto Laser assisted Chop & Cylinder pattern of pretreatment. In group 3, 40 eyes (40 patients) underwent 2.0 mm MICS. All three groups underwent phacoemulsification on Centurion Vision System and CDE values were evaluated. Pre and post-operative Central Corneal Volume (3mm) and Pentacam Nucleus Grading (4.00) by Pentacam HR were measured. Pre-operative, 7 days and 30 days post-operative endothelial count were measured by Specular Microscopy SP-3000P, Topcon.

Results:

Tukey Post Hoc Analysis showed significance among the three groups for CDE count, Group 1 and Group 2 (p=0.000); Group 1 and Group 3 (p=0.000) and Group 2 and Group 3 (p=0.000). 1st day post-operative CCV (3mm) was lesser in Lensx Fragmentation group (63.13±3.57) as compared to groups 2 (63.42±10.2) and group 3 (67.97±4.38) and results were statistically significant (p=0.036) and 30th day Post operative CCV were 61.61±3.45: 62.29±4.27 and 62.5±4.03 respectively (p=0.130) Post operative Corneal Endothelial cell count were found to be lower in MICS group at 1st and 30th day but difference were not statistically significant (p=0.31)

Conclusions:

400 micron Femtosecond laser grid pattern (group 1) and Chop Cylinder pattern (group 2) pretreatment allowed for a significant reduction in Cumulative Dissipated Energy compared to Micro Incision Cataract Surgery (group3) for denser lens nuclei. This has resulted in less endothelial cell trauma and significantly less central corneal swelling on first post operative day in 400 micron Femtosecond laser grid pattern (group 1) and Chop Cylinder pattern (group 2) pretreatment as compared to Micro Incision Cataract Surgery (group 3) without any Laser pre treatment of Dense Nucleus.
