To present a differential diagnosis of keratoconus in one case of long term pseudokeratoconus due to an epithelial irregularity

A fifty-four-year old patient with a history of loss of vision in the last 4 years in the right eye and keratoconus diagnosis. UCVA was 20/100 and BCVA was 20/25 with a refraction of (-1 -3 x 90ยบ). The left eye was plano with 20/20 vision. Anterior corneal topography showed a keratoconus image in the right eye. Slit-lamp examination revealed a subepithelial abnormality in the pupillary area. Corneal OCT showed the epithelium not well adhered to the basal membrane and the corneal thickness was normal. We established the diagnosis of epithelial pseudokeratoconus induced by an asymptomatic recurrent corneal erosion syndrome


We treated this case by mechanical epithelium debridement and patching with a therapeutic contact lens until complete re-epithelialization. One month later UCVA was 20/20 and corneal topography was normal


A differential diagnosis between keratoconus and epithelial pseudokeratoconus must be taken into account because any irregularity of the cornea could induce topography and refraction similar to keratoconus
