The utility of colistin in multiple drug-resistant Pseudomonas aeruginosa bacterial keratitis in a Kaposi’s sarcoma patient

Abstract Details

Purpose:

To report the therapeutic outcome of colistin for multiple drug-resistant Pseudomonas aeruginosa bacterial keratitis in one-eyed, Kaposi’s sarcoma patient.

Setting:

Ege University Hospital, Ophthalmology Department, Bornova - Izmir / TURKEY

Methods:

A 71-year-old male patient was presented with decreased visual acuity, redness and discharge in his right eye for 5 days. He had an evisceration surgery for his left eye several years ago. At the time of diagnosis, he was also receiving treatments for scalp Kaposi’s sarcoma. Slit-lamp exam revealed an extended corneal ulcer with surrounding infiltrate, which was extended to the deep stroma with severe hypopyon.

Results:

Microbiological evaluation identified the causative agent to be multiple drug-Resistant Pseudomonas aeruginosa. Based on culture and sensitivity reports, the patient was started on hourly instillation of topical colistin 0.19%. A complete resolution of keratitis with residual scarring was noticed.

Conclusions:

In recent years, there is an increase in drug resistance in P. aeruginosa keratitis. Multiple drug-resistance P. aeruginosa is particularly a problem among the critically ill patients. The lack of new antimicrobial agents against these resistant ones has led clinicians to the reconsideration of colistin, which is an old drug. Hereby we would like to stress the utility of colistin in multiple drug-resistant Pseudomonas aeruginosa bacterial keratitis in a Kaposi’s sarcoma patient with this case report.
