Abstract Details

Purpose:

Controversy exists not only regarding the decrease of contrast sensitivity values after the implantation of a multifocal intraocular lens, but also by the presence of glistenigns within the IOL. We tested the hypothesis that the amount of glistening is related with the decrease of contrast sensitivity. We performed a retrospective cohort study to assess long term visual outcomes after the implantation of Restor + 3.00 multifocal intraocular lens at our institution.

Setting:

Instituto de Oftalmología ,Fundación Conde de Valenciana, Mexico City, Mexico.

Methods:

We analyzed 36 of 18 randomly selected patients with previous implantation of Restor +3.00 since 2008 (2008-2014) at Instituto de Oftalmología Fundación Conde de Valenciana, Anterior segment department. We analyzed visual acuity (ETDRS) at near, far and intermediate distance, contrast sensitivity test (FACT), visual function questionnaire (VFQ-25) PSF values (sterhl ratio) and grade of glistenings. We show all results and correlate level of glistenings with visual acuity and contrast sensitivity values.

Results:

We described results from 36 eyes. 88.2% of our study population developed some grade of glistening within the multifocal intraocular lens. 82.4%. The range of vision in our patients were as follows : 0.0 to 0.4 Log MAR in far vision, 0.0 to 0.5 Log MAR at intermediate vision (80cm) and from 0.0 to 0.20 Log MAR at near vision (33 cm). We did not found nay correlation with glistenigns and visual acuity. However we did found significant reduction in contrast sensitivity values, specially at medium and high frequencies when correlated to higher grades of glistening (+3 or +4 at the Miyata's scale) (p<0.001). None of the patients were linked to low VFQ25 values.

Conclusions:

The restor +3.00 multifocal intraocular lens is a good option for patients with presbyopia and cataract . Most patients remain satisfied and glistenings seems to decrease mainly contrast sensitivity values. It is always better to inform patients that they may experience this side effect.
